Saw palmetto on its own, at a stated dose, sold with the two large trials that went against it rather than around them.

What it's for

Saw palmetto berry extract is the most-used botanical for prostate and urinary comfort in older men. You should know how the evidence actually landed: the two largest, best-run trials — STEP and CAMUS — compared it against placebo, including at doses well above the usual one, and found no meaningful difference in urinary symptoms. Earlier, smaller studies were more positive. The honest summary is that when the trials got bigger and better controlled, the effect shrank towards nothing.

How to take it

  • Follow the label serving, with food — it is fat soluble and absorbs poorly on an empty stomach
  • Give it two to three months before drawing any conclusion
  • May have a mild blood-thinning effect. Mention it before surgery and if you take an anticoagulant
  • See a doctor about the symptoms, not just this bottle. Difficulty passing urine, going frequently at night, a weak stream or blood in the urine all need a proper assessment. Those symptoms are usually a benign enlarged prostate — but they can also be prostate cancer, and taking a supplement instead of getting checked is how a diagnosis gets delayed
